How do I upload a copy of my health insurance card for the Student Health Center?


CLICK HERE to upload a copy of your insurance card via the Healthy Coog Patient Portal. We accept the following file types: PNG, GIF, JPEG, JPG. 


It is not necessary to upload your insurance information if you are enrolled in the UH System-endorsed Student Health Insurance Plan. Your information is automatically uploaded into our Electronic Medical Records within 48 hours of you enrolling in the plan.
