Why have I have stopped receiving push notifications on Duo mobile app? 

If you have stopped receiving push notifications, then check for network between your phone and internet. It may help to take your phone into and out of airplane mode. If there is not a network problem, then request a re-activation of the service from the UIT Support Center via live chat or 713-743-1411.
