Checks must be payable to the University of Houston only.

Check or accompanying donor documentation must include the student's name and Social Security Number. Include semester for which funds are to be used.

An official memo on business letterhead must be included from the donor indicating additional requiremens, if necessary. These would include, but not be limited to enrollment in a specific number of hours or if there is no criteria necessary.
